What is Family Navigation

Our support is designed to be short-term and focused, meeting families where they are and responding to their most immediate needs. Services may take place in one session or a small number of follow-ups, depending on the situation.

Participation is always confidential, judgment-free, and rooted in respect and dignity.

Who This Service Is For

Family Navigation is available to:

  • Families seeking guidance or resources

  • Parents and caregivers navigating community systems

  • Individuals needing short-term assistance or longer-term support

  • Families already connected to other AHCP programs

Families choose how much support they want — there is no obligation to continue services.

How We Support Families

Short Term Family Navigation Support
(Case Management)

After Hours Community Project provides short-term family navigation support for individuals and families who need immediate, focused assistance.

Our services are designed to help families understand their options, complete next steps, and connect to the right resources — without requiring long-term case management.

Short-term family navigation may include help with:

  • Completing applications and forms

  • Understanding notices, paperwork, or eligibility steps

  • Navigating public benefits such as SNAP, Medicaid, CHIP, or TANF

  • Resume support or basic job-readiness assistance

  • Connecting to a specific community resource or referral

  • General guidance, problem-solving, and next steps


Support is typically provided in one session or a small number of follow-ups, depending on the family’s needs.

How Family Navigation Works

Family Navigation at After Hours Community Project is designed to be simple, supportive, and easy to access.

  1. Reach Out
    Families contact AHCP through our website, phone, or in person to request support.

  2. Brief Intake
    We complete a short intake to understand the family’s immediate needs and how we can best help.

  3. Focused Support Session
    A Family Navigator provides short-term assistance, which may include:

  • Help understanding applications or paperwork

  • Guidance on public benefits and community resources

  • Support identifying next steps and available options

  1. Next Steps & Referrals
    Families leave with clear next steps and referrals to additional resources when needed.

  2. Optional Follow-Up
    A brief follow-up may be offered to check progress or answer additional questions.

How to Get Started

Getting started is simple. Families can request short-term assistance or ongoing support directly, or may be referred through an AHCP program or community partner.
